Findings
The Alleged Victim (AV) relies on the facility for his/her personal care. AV has a history of refusing showers. According to an investigation, on or about April 25, 2025, AV was found unkept, with stool up AV’s gown, material stuck between AV’s toes and had an odor. Between March 3, 2025 and April 25, 2025, AV refused showers approximately fourteen times. The facility’s failure to care plan appropriately, which is a violation of resident rights and is considered neglect which constitutes abuse.
