Findings
The Alleged Victim (AV) relies on the facility to provide care, services and a safe environment. According to the investigation, AV was known to be smoking in their room multiple times by facility staff, but there were no safety plans or updates to the care plan to provide interventions to help prevent AV from smoking in their room. AV sustained multiple burns and suffered toe amputations from the burns. The facility failed to properly care plan interventions for AV, which is a violation of resident rights and is considered neglect, which constitutes abuse.
