- On January 9, 2026, ODHS Adult Protective Services (APS) completed investigation #00398468 and issued a written investigation report, which is incorporated here by reference. Alleged Victim (AV) lived at Respondent’s facility. Based on facility documentation and interviews, on or about April 30, 2025, when Alleged Perpetrator 2 (AP2) was assisting AV out of his/her room, AV stopped to take a break from walking. In response, AP2 then grabbed AV’s groin area in an attempt to get AV to continue walking. As a result of AP2’s action, AV experienced unreasonable discomfort. AP2 used physical force to compel AV to continue walking which is constitutes physical abuse. The facility was not substantiated for abuse by the APS investigator; however, the facility failed to provide a safe environment for AV which is considered a licensing violation of Oregon Administrative Rules.
