- The Alleged Victim (AV) was identified upon move in to the facility on December 17, 2024 to have broken molars and in need of dental care. AV's care plan stated that staff were to assist AV with brushing of teeth. AV's care plan also noted that AV was to be assisted with medical appointments and transportation. On or about April 29, 2025, AV complained that he/she had a level 10 tooth pain. AV was given pain medication for chronic pain, however, AV's dental pain was not alleviated. AV had stated that he/she was having dental pain for approximately a week. On or about May 5, 2025, AV determined that he/she wanted to be sent to the emergency room (ER) due to tooth pain. AV was sent to the ER on May 5, 2025 and diagnosed with a dental infection and a closed fractured tooth. The facility's failure to ensure proper and timely dental care to AV, causing repeated unreasonable discomfort, is a violation of resident rights, is considered neglect of care and constitutes abuse.
