Findings
On January 7, 2026, ODHS Adult Protective Services (APS) completed investigation #00399604 and issued a written investigation report, which is incorporated here by reference. The Alleged Victim (AV) relies on the facility for care, supervision, and safety. AV is diagnosed with a major neurocognitive disorder and has some disorientation and deficits in judgement. AV's service plan reflected AV ambulates independently without an assistive device and at risk of elopement. On or about May 3, 2025, facility staff discovered AV was missing from the facility. Based on facility documentation and interviews, AV had eloped from the secured facility through a broken section of the facility's fence and was able to ambulate 1.5 miles away from the facility on a high-use road and necessitated AV crossing intersections. The APS investigation determined that the facility failed to provide appropriate supervision to the AV according to his/her care needs. The failure resulted in AV eloping the secured building, without staff knowledge, placing him/her at serious risk of harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
