Findings
Based on review of facility documentation and interviews, it was determined the facility failed to ensure a safe medication administration system for Alleged Victim (AV), a resident of the facility. On or about May 10 and May 11, 2025, AV missed two doses of prescribed thyroid medication. Documentation shows AV’s family requested that the medication be packed for an outing; however, the facility did not ensure the medication was provided and administered as ordered. The facility’s failure is a violation of Oregon Administrative Rules governing medication administration. AP2 allegedly neglected AV. An investigation determined no abuse occurred by AP2.
