Findings
The Alleged Victim (AV) relies on the facility for medication management. According to an investigation, on or about May 9, 2025, AV was diagnosed with an infection and prescribed antibiotics. Staff did not know the after-hours procedure for getting urgent medications, which resulted in AV not receiving the medication for several days. AV’s infection got worse, resulting in AV going to the hospital for treatment on or about May 12, 2025. The facility failed to provide a safe medication administration system, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
