Findings
The Alleged Victim (AV) is a known fall risk. On or about May 7, 2025, AV suffered a stroke. AV had suffered falls prior to his/her stroke. On or about May 14, 2025, AV suffered a fall from his/her bedside commode, suffering bleeding and swelling to AV's face, nose and lips. AV had a history of falling from his/her commode in the past. The facility's failure to have interventions in place to ensure AV's safety while using the commode is a violation of resident rights, is considered neglect of care and constitutes abuse.
