- The Alleged Victim (AV) is a known fall risk and has suffered falls with significant injury in the past. On or about May 18, 2025, AV had an unwitnessed fall in his/her room, which resulted in a large bump to his/her head. At the time of the fall, AV was stand by assist with ambulation with the use of his/her walker or cane. AV was on two hour safety checks and was known to attempt to walk without assistance or use of the walker or cane. The facility's failure to ensure proper interventions were in place to reduce AV's risk of falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
