Findings
Between March 2025, and May 2025, it was discovered that 4 doses of the Alleged Victim's (AV) narcotic medication were crossed out on the narcotic log by Alleged Perpetrator #2 (AP2) and Alleged Perpetrator #3 (AP3). Facility records do not indicate that AP2 or AP3 were trained on medication administration. AV missing his/her narcotic medication left AV in unreasonable discomfort. The Allegations that AP2 and AP3 financially abused AV is found to be inconclusive. Respondent failed to ensure staff were properly trained and provide a safe medication administration system. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
