Findings
On or about April 3, 2025, it was discovered that 1 dose of the Alleged Victim's (AV) narcotic medication were crossed out on the narcotic log by Alleged Perpetrator #2 (AP2). There is no record of a disposition of AV's missing medication. The facility was not performing regular audits of resident medications, including controlled substances. Facility records do not indicate that AP2 or Alleged Perpetrator #3 (AP3) were trained on medication administration. The Allegations that AP2 financially abused AV is found to be inconclusive. Respondent failed to ensure staff were properly trained and provide a safe medication administration system. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
