Findings
The Alleged Victim (AV) requires assistance with bladder management. Between March 2025 and June 2025, AV waited multiple times for staff assistance, ranging from 20 minutes to over an hour. This long wait caused AV unreasonable discomfort and placed AV at risk for skin breakdown. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
