- According to the documentation, the facility failed to follow the care plan to ensure meat was cut up into pea sized bites for the Alleged Victim (AV) on or about May 27, 2025, the AV was served a meatloaf sandwich for lunch and was cut into bite sized bites when their service plan stated they needed meat to be cut into pea sized bites. As a result of not following this plan, the AV choked on their food and needed the Heimlich maneuver to dislodge the food. The failure to follow the care plan placed the resident at risk for serious harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
