- On or about June 11, 2025, the Alleged Victim (AV) was served sausage during a mealtime, despite having a documented allergy to processed meats. Due to AV’s impaired condition, AV is unable to independently identify foods that are unsafe to consume. Based on facility documentation and staff interviews, it was determined that the facility failed to provide the basic care and services necessary to prevent AV from being served allergenic food. This failure to follow AV’s specific dietary orders and service plan resulting in AV experiencing unreasonable discomfort due to labored breathing and swelling in the hands and feet which are symptoms consistent with a potentially life-threatening allergic reaction. The facility's failure to adhere to AV's care plan is violation of resident rights, is considered neglect of care and constitutes abuse.
