- The Alleged Victim (AV) relies on the facility to provide a safe environment. According to an investigation, on or about June 16, 2025, Alleged Perpetrator 2 (AP2) and Alleged Perpetrator 3 (AP3) saw former staff members inside the facility. The facility policy is that former staff members are not allowed in the facility without prior management approval. AP2 did not follow proper protocol in removing the former staff members from the premises. AP3 was responsible for locking the front door and did not do so and did not ask the former staff members to leave the premises. The former staff were allowed to gain access into AV's room. The facility failed to provide a safe environment which violates Oregon Administrative Rule. AP2 and AP3 failed to provide a safe environment, which is violation of resident rights and considered neglect, and constitutes abuse.
