- Witness #1 (W1) has known aggressive and combative behaviors and is care planned to have cueing and redirection during meal times, as W1 is known to get up and take other residents plates. On or about June 20, 2025, W1 took another residents plate at meal time and the Alleged Victim (AV) attempted to remove the plate from W1 and W1 responded by slapping AV. AV was not injured, however, AV was placed at risk for harm. Facility staff did not intervene timely to ensure the safety of other residents. The facility's failure to follow W1's care plan is a violation of resident rights, is considered neglect of care and constitutes abuse.
