Findings
The Alleged Victim (AV) relies on the facility and Alleged Perpetrator 2 (AP2) to provide a safe medication administration system. According to an investigation, on or about June 3, 2025, AP2 accidentally discontinued AV's medication and did not have a physician's order to stop the medication. AV went without medication for approximately twenty days before it was noticed and then administered to the resident. The facility and AP2 did not provide a safe medication administration system for AV, which is a violation of resident rights and are considered neglect of care, which constitutes abuse.
