Findings
The Alleged Victim (AV) relies on the facility for his/her care. According to an investigation, on or about July 13, 2025, Alleged Perpetrator 2 (AP2) left a meal tray in AV’s room at approximately 4:45pm without having a visual on AV. AV was discovered missing at approximately 8:00pm during a medication pass. and confirmed that AV left the facility at approximately 3:30pm. AV was picked up by EMS at approximately 7:00pm about two-three blocks from the facility and transported to the hospital with heat exhaustion. AP2's actions are considered neglect and constitutes abuse. The facility failed to provide a safe environment, which is a violation of Oregon Administrative Rules.
