- The Alleged Victim (AV) relied on the facility for care. The AV developed pressure wounds in June 2025. During a July 4, 2025, hospital visit the wounds were noted to be on AV’s lower back, buttocks, and AV’s heel, and interventions were added to help AV heal. On or about July 22, 2025, AV's dressing was found to be drenched with wound drainage only 10 minutes after it was documented that the area was dry. According to an investigation, the facility repeatedly failed to follow the interventions and AV’s wound increased in size and became infected. The facility failed to provide services, to include implementing interventions and / or properly care planning, and responding to AV’s change of condition and monitoring, resulting in repeated unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
