- Based on witness statements, investigator observations, and documentary evidence collected during Adult Protective Services (APS) Investigation #00416218, APS Investigator determined that on or about July 25, 2025, Alleged Perpetrator 2 (AP2) used their forearm to shove the Alleged Victim’s (AV) head, causing AV unreasonable discomfort. At time of incident, AP2 was administering medications to AV and acted with excessive physical force. AP2 had received training on the appropriate treatment of residents and was expected to de-escalate or remove themselves from the situation. AP2's use of physical force resulting in unreasonable discomfort to AV is considered physical abuse which constitutes abuse. The facility failed to protect AV from AP2's actions, which is a violation of Oregon Administrative Rules.
