- The Alleged Victim (AV) requires care of an ostomy bag by facility staff. Alleged Perpetrator #3 (AP3) has trained Alleged Perpetrator #2 (AP2) on how to care for and change AV's ostomy bag. On or about July 29, 2025, AP2 was caring for AV's ostomy bag and changing it out for a new one. After AP2 completed this task, AV advised AP2 that the bag was cut incorrectly and it was hurting AV. AP2's response was "it's the best i can do" and walked away from AV. AV was found to have been cut by the improperly cut bag. AP2's failure to provide correct ostomy care to AV caused a cut and unreasonable discomfort to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
