Findings
The Alleged Victim (AV) is dependent on the facility to meet his/her safety needs and has a history of frequent falls. According to an investigation, on or about July 25, 2025, AV was found on the floor with an injury to their head. The AV was sent to the hospital and diagnosed with a fractured arm. AV's service plan was last updated on 6/5/2025. Between 6/5/25 and 7/25/25, AV experienced at least six additional falls. The facility failed to ensure AV's service plan was reviewed and updated as needed with interventions. The facilities failure to properly plan care resulted in falls with injury and a fractured arm, which is a violation of resident rights, is considered neglect of care which constitutes abuse.
