Findings
Alleged Victim (AV) has a known history of elopement. On or about August 1, 2025, AV exited the memory care unit unattended. AV had gotten out of the door by pressing the lever rapidly which causes the door to open after 15-30 seconds. AV was at risk of being taken advantage of in the community or walking into the road. Respondent failed to implement meaningful interventions for elopements, to ensure the safety of AV. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
