- The Alleged Victim (AV) suffered two falls on August 4, 2025 and was sent to the hospital and was found to have a bruise on his/her buttocks. On or about August 4, 2025, it was discovered that Alleged Perpetrator #2 (AP2) had given AV a sedative to calm him/ her down. AV did not have any as needed medication listed as an intervention. AP2 gave the medication for his/her convenience, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure to ensure medication is not used as a restraint is a violation of Oregon Administrative Rules.
