Findings
The Alleged Victim (AV) relies on the facility to provide care and services. According to an investigation, on or about August 4, 2025, AV suffered an unwitnessed fall. AV is care planned for safety checks to be done every two hours and to have their door open. Alleged Perpetrator 2 (AP2) was assigned to AV did not check on AV in the scheduled two-hour time limit. The facility failed to provide care and services, and this is a violation of Oregon Administrative Rules. AP2 failed to provide care and services to AV which is a violation of resident rights, considered neglect which constitutes abuse.
