Findings
The Alleged Victim (AV) relies on the facility for his/her care and to administer his/her medications. According to an investigation, on or about July 27, 2025, and over a period of at least 15 days, AV did not receive antipsychotic medication as ordered. AV was administered the medication once per week and not once per day as ordered. The medication order had been transcribed incorrectly and the error was not corrected by the three-check system in place at the facility. The facility failed to ensure medication was administered as ordered, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) neglected AV was investigated and determined to be not substantiated.
