Findings
Alleged Victim (AV) lives at Respondent’s facility. AV service plan reflects AV relies on facility staff to order, store, and safely administer all medications. AV is at risk of strokes if AV's blood pressure medication in missed. AV went without AV's blood pressure medication from August 12, 2025, to August 15, 2025. Respondent staff failed to follow AV service plan, and ensure AV's medications were properly ordered and available for AV. This constitutes abuse by neglect as defined by OAR 411-020-0002(1)(b)(A)(i).
