- Alleged Victim (AV) lives and relies on the Respondent's facility staff to ensure AV's basic care, safety and supervision requirements are met. Alleged Perpetrator #2 (AP2) is a facility staff member employed by the Respondent and is responsible to provide care to AV. AP2 received and acknowledged receipt of trainings necessary to provide care to residents. On or about August 21, 2025, AP2 grabbed AV's wrists and held them down against AV's chest. AP2’s failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility failure is a violation of Oregon Administrative Rules.
