Findings
The Alleged Victim (AV) relies on the facility staff their care and to ensure resident’s rights. According to an investigation, on or about August 10, 2025, Alleged Perpetrator 2 (AP2) yelled at AV, resulting in emotional distress to AV. AP2's action is considered neglect and constitute abuse as defined in OAR 411-020-0002(1)(b)(A)(i). The facility failed to ensure resident rights and to protect AV from verbal/emotional abuse, which is a violation of Oregon Administrative Rules.
