Findings
On or about July 27, 2025, the Alleged Victim (AV) was found outside in the courtyard on the ground by a family member. When residents exit the facility and go into the courtyard, staff are supposed to watch residents in the courtyard. On this day, no staff were watching the courtyard. AV fell in the courtyard and suffered a skin tear to his/her arm. The facility's failure to ensure a safe environment and supervision of the courtyard is a violation of resident rights, is considered neglect of care and constitutes abuse.
