Findings
The Alleged Victim (AV) relies on the facility for care and services. AV has a food allergy that is in AV's service plan. The facility staff are to ensure that AV is not exposed to food allergens. During an event at the facility, staff served AV a dish that had food in it that AV is allergic to. This caused AV to need to use an EPI-pen and then go to the hospital for further evaluation due to the allergic reaction AV had to the food. The facility failed to follow the care plan for AV, which is neglect of care and constitutes abuse.
