Findings
The Alleged Victim (AV) relies on the facility for his/her care. AV was care planned for physical assistance with all ambulation/mobility needs. According to an investigation, on or about August 29, 2025, AV experienced a fall while attempting to self-ambulate to the bathroom, resulting in pain. The facility failed to follow the care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
