Findings
The Alleged Victim (AV) relies on the facility for their care, including medication management. According to the investigation, AV did not receive their medication on approximately two occasions between September 17, 2025, and September 18, 2025, resulting in a seizure. The facility failed to provide a safe medication management system which is a violation of resident rights, is neglect of care which constitutes abuse.
