Findings
Alleged Victim (AV) lives at Respondent’s facility. AV's service plan reflects facility will store, administer all medications per PCP orders. Facility staff are to order medications 7 days prior to medication running out. On or about September 14, 2025, AV did not receive h/h scheduled behavioral health medication because the facility did not have it available. AV went without h/h behavioral health medication from September 14, 2025, through September 19, 2025. Missing six consecutive doses was likely to cause AV behavioral changes and instability. Respondent failed to provide a safe medication administration system to ensure AV received medication as ordered, which constitutes abuse by neglect.
