Findings
The Alleged Victim (AV) relies on the facility for his/her care. AV has a history of exit seeking behavior and a prior elopement. According to an investigation, on or about September 26, 2025, AV eloped the facility, posing a risk of harm. The facility failed to provide a safe environment, which is a violation of resident rights, is neglect of care and constitutes abuse.
