Findings
Alleged Victim (AV) lives at Respondent’s facility and relies on the facility staff to provide appropriate supervision and care. AV has a history of falls, including two prior incidents that occurred in July 2025, that resulted in EMS transporting AV to the hospital for evaluation. However, the facility failed to develop sufficient interventions to mitigate risk of further falls to AV. On or about August 10, 2025, AV was found on the floor with a skin tear to right skin fold and redness to right knee. AV also complained of hip pain. Respondent failed to develop and implement suitable interventions, which constitutes abuse by neglect.
