- On January 21, 2026, ODHS Adult Protective Services (APS) completed investigation #00433403 and issued a written investigation report, which is incorporated here by reference. Alleged Victim (AV) lived at Respondent’s facility. Based on facility documentation and interviews, on or about October 14, 2025, AV ran out of his/her restless leg syndrome medication and was without this medication until October 17, 2025. The facility is responsible for managing AV’s medications, including monitoring supply levels and ensuring timely reordering. Based on facility documentation and staff interviews, the facility did not order AV’s medication for restless leg syndrome in a timely manner, resulting in missed doses, causing AV unnecessary pain and discomfort which is a violation of resident's rights, is considered neglect of care, which constitutes abuse.
