Findings
Alleged Victim (AV) lives at Respondent’s facility. AV has history of an elopement attempt, including an incident that occurred in September 2025, when AV watched the code by the door and knew it started with a "7", and attempted to follow a facility staff member out through the exit door. On or about October 19, 2025, AV exited the locked memory care unit by inputting the secure code. Respondent failed to develop, implement interventions, and provide supervision, and safety, resulting in AV eloping, which constitutes abuse by neglect.
