Findings
The Alleged Victim (AV) relies on the facility for their care. AV has a diagnosis of a neurodegenerative disease that affects AV's gait and balance, as well as having memory impairment. AV has a history of falls, is at a high risk for falls and is considered a frequent faller. According to an investigation, on or about October 4, 2025, AV fell in the bathroom, resulting in upper back pain. The facility failed to provide appropriate services related to properly planning care and/or implementing interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse as defined in OAR 411-020-0002 (1)(b)(A)(i) and (ii).
