- The Alleged Victim (AV) is a known wanderer in the community. AV wanders into Witness #1's (W1) room regularly. On or about October 18, 2025, AV wandered into W1's room during the middle of the night. W1 pressed his/her call pendant for staff assistance. When staff arrived, they found AV on the floor of W1's room. AV was not injured from the interaction. AV's care plan does not indicate interventions for AV's wandering, although staff attempt to keep an eye on AV at night. The facility's failure to have interventions in place regarding AV's wandering placed residents at risk for harm, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
