- The Alleged Victim (AV) is identified to have cognitive decline, known to wander and exit seek, and requires a secured living environment. According to an investigation, on or about October 21, 2025, the AV eloped from the secured facility. The AV walked for approximately thirty-five (35) minutes on a heavily trafficked roadway, placing them at risk of harm. The facility failed to ensure a safe environment for the AV, which is a violation of resident’s rights, is neglect of care, and constitutes abuse.
