Findings: The Alleged Victim (AV) relies on the facility for their care. AV had an order to be checked every hour and apply barrier cream and was known to refuse cares. According to an investigation, on or about October 27, 2025, AV was found with AV’s brief, pad and bedding soaked with urine. AV developed skin reddening and breakdown. The facility failed to provide services, to include properly care planning and/or implementing interventions and following medication and treatment orders, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP26-00261 $500.00 fine assessed
