Alleged Victim (AV) relies on the facility for his/her care. Fraudulent activity was identified on two of AV’s credit cards during the period of 8/2025-10/2025. Alleged Perpetrator 2 (AP2) admitted to writing down two of AV’s credit cards information. AP2 made multiple fraudulent charges between 8/2025-10/2025. AV experienced a total financial loss of $1.973.28. AP2 failed to protect AV from financial exploitation, which is neglect of care and constitutes abuse. The facility failed to protect AV from financial exploitation, which is a violation of Oregon Administrative Rules.
