- On or about November 1, 2025, the Alleged Victim (AV) eloped from the community through a side door that did not latch, resulting in AV being outside the locked community for approximately 7-8 minutes. AV was a new admission to the facility and was not deemed an elopement risk. Staff did not recognize AV outside of the community and allowed AV to wander in the parking lot to the other side of the building and re-entering the facility through the kitchen door. The community is near a major highway, therefore, AV eloping from the facility places AV at risk for serious harm. Respondent failed to provide a safe environment, due to a door with a faulty latch, allowing AV to exit the community without supervision. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(ii).
