- On or about November 9, 2025, the Alleged Victim (AV) was found to have three very large water blisters on his/her legs. Alleged Perpetrator #2 (AP2) was notified by staff of the blisters but did not take any actions for AV's wounds. On or about November 15, 2025, AV was sent to the hospital for evaluation of the blisters and prescribed medication and advised to follow up with AV's physician. AV's physician saw AV on November 17, 2025, and called home health, as the facility failed to contact home health to come in to complete wound care. AP2's actions are a violation of resident rights, are considered neglect of care and constitute abuse. The facility's failure to ensure AV received proper nursing services is a violation of 411-020-0002(1)(b)(A)(i), which constitutes abuse by neglect.
