Findings
The Alleged Victim (AV) resides at the facility and relies on the facility to meet his/her daily needs. According to an investigation, on or about November 24, 2025, it was discovered the Alleged Perpetrator 2 (AP2) had been involved in a sexual relationship with AV while AP2 was employed by AP1 and AV was a resident. Communication ended between AV and AP2, resulting in AV feeling emotionally upset, crying and worried. AP2's actions are a violation of resident's rights, are considered neglect of care and constitutes sexual abuse. The facility failed to protect AV from sexual abuse, which is a violation of Oregon Administrative Rules.
