Findings
On or about November 25, 2025, the facility failed to implement appropriate fall interventions related to the Alleged Victim’s (AV) known fall history which resulted AV experiencing a fall that resulted in a hip fracture causing AV pain and unreasonable discomfort. The facility’s failure to develop sufficient fall interventions to mitigate risks to AV is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
