- Alleged Victim (AV) is care planned to have housekeeping assistance and to have AV’s bed made daily. AV uses a four-wheeled walker for ambulation. On or about December 06, 2025, social support went to support AV in the home after AV was observed smelling like urine. Social support noted AV's room to not have clear walkways, rotting food, clothing, and other items all over AV's room. Social support noted that AV was at increased risk for tripping/falling due to clutter and lack of space. AV's mattress had no sheets on it and was "unmade" and social support noted when they lifted AV's mattress, urine seeped/dripped off of AV's mattress. The carpet in AV's room was heavily soiled and stained and AV's room had an odor to it. AV's social support reported finding sharps (lancets and needle tops) all over AV's floors and cleaned those prior to remember to take photographs but noted that AV was not effectively using sharps containers for disposal. Respondent failed to provide service, and to ensure the safety of AV. This constitutes abuse by neglect, as outlined in OAR 411-020-0002(1)(b)(A)(i).
