Findings
On or about December 14, 2025, Alleged Perpetrator #2 (AP2) came up behind the Alleged Victim (AV), who was sitting in their wheelchair and began pushing AV in his/her wheelchair down a hallway that was not towards his/her room. AV stated "ouch" and "no" to AP2 and put his/her feet down to stop the movement of the wheelchair. AV is independent with his/her wheelchair and does not liked to be pushed. AP2's actions caused AV unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure to protect AV from unreasonable discomfort is a violation of Oregon Administrative Rules.
