- The Alleged Victim (AV) relies on the facility for their care. AV is a high fall risk with a history of falls. AV is known to walk away from their walker. AV is care planned for staff to provide full assistance with walking needs and to remind AV to use assistive devices and if seen without walker. According to an investigation on or about December 27, 2025, AV was found lying on the floor on AV’s left side in the common area, bleeding from the front of the head. AV did not have AV’s walker or wheelchair next to them. AV was seen previously walking around the community appearing anxious. AV was sent to the hospital and returned with staples to the forehead. The facility failed to provide appropriate services, relating to care planning, following the care plan and/or implementing appropriate interventions, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
